The TSH-Only Problem

TSH (Thyroid Stimulating Hormone) is a pituitary hormone — it measures how hard your brain is pushing your thyroid, not how much thyroid hormone you actually have. A normal TSH can coexist with low Free T3, the active hormone your cells actually use. Checking only TSH is like checking your car's gas gauge without looking at the fuel injectors.

The T4→T3 Conversion Failure

T4 (levothyroxine, Synthroid) is an inactive storage hormone. Your body must convert it into T3 to use it. Many patients — especially under stress, with inflammation, or nutrient deficiencies — convert T4 poorly, producing excess Reverse T3 instead. Standard testing doesn't check this conversion, so it gets missed entirely.

The Hashimoto's Detection Gap

Hashimoto's thyroiditis — the most common cause of hypothyroidism — is an autoimmune attack on thyroid tissue. Early Hashimoto's can produce normal TSH and T4 levels while destroying thyroid function. It's only detectable through TPO and thyroglobulin antibody testing, which most standard panels don't include.

Levothyroxine (T4)

For patients who convert T4 to T3 efficiently, levothyroxine is appropriate — but dosed to optimize Free T4 and T3 levels, not just TSH.

Liothyronine (T3)

When conversion is impaired

Direct T3 supplementation bypasses the conversion step entirely. Often prescribed in combination with T4 for patients with high Reverse T3 or poor T4-to-T3 conversion.

Natural Desiccated Thyroid (NDT)

For whole-hormone support

NDT (like Armour or NP Thyroid) contains both T4 and T3 in a natural ratio. Many patients who don't feel well on T4-only therapy respond dramatically to NDT.

Every cell in your body has thyroid hormone receptors. Thyroid doesn't just control your temperature — it regulates how efficiently your cells burn energy, how your heart beats, how your brain functions, how your gut moves, and how well your sex hormones work.

This is why hormone therapy and weight loss medications often underperform for patients with unaddressed thyroid dysfunction. Testosterone requires adequate T3 to work properly. GLP-1 medications are less effective when thyroid is suppressing metabolism. HRT results are blunted when thyroid-related fatigue and brain fog aren't addressed first.

My TSH came back normal. Could I still have a thyroid problem?+

Yes — and this is one of the most common frustrations we hear. TSH is a pituitary hormone, not a thyroid hormone. A normal TSH means your pituitary is satisfied, but it says nothing about whether your thyroid is actually producing or converting enough T3 (the active hormone). We test Free T3, Free T4, Reverse T3, and thyroid antibodies — the markers that actually tell the full story.

What's the difference between T4 and T3, and why does it matter?+

T4 (thyroxine) is the storage form of thyroid hormone — mostly inactive. Your body must convert T4 into T3 (triiodothyronine), the active form that actually fuels your cells. Many people with normal T4 levels have poor T4-to-T3 conversion, meaning their cells are still functionally hypothyroid. Stress, inflammation, low iron, and certain medications all impair this conversion.

What is Hashimoto's thyroiditis and do you test for it?+

Hashimoto's is an autoimmune condition where the immune system attacks thyroid tissue. It's the most common cause of hypothyroidism and is frequently missed because standard TSH-only testing doesn't include thyroid antibodies. We test TPO and thyroglobulin antibodies to identify Hashimoto's — because the treatment approach is different when autoimmunity is involved.

What medications do you prescribe for thyroid optimization?+

We prescribe what your body actually needs — not just what's easiest. For patients who convert T4 to T3 well, levothyroxine (T4-only) may be appropriate. For patients with conversion issues, we may prescribe combination T4/T3 therapy (liothyronine), or natural desiccated thyroid (NDT) which contains both hormones in a natural ratio. Protocol depends entirely on your labs.

Can thyroid imbalance cause weight gain even when I'm eating well?+

Absolutely. Low thyroid function — even subclinical hypothyroidism — reduces basal metabolic rate, impairs fat metabolism, causes water retention, and increases insulin resistance. Patients with undertreated thyroid dysfunction often find that weight loss is nearly impossible until thyroid function is optimized, even with perfect diet and exercise.

How does thyroid connect to my hormone therapy?+

Thyroid and sex hormones are deeply interrelated. Low T3 reduces the effectiveness of estrogen and testosterone at the cellular level — meaning HRT and TRT can underperform until thyroid is addressed. We routinely find that patients who weren't getting full results from hormone therapy see dramatically better outcomes once thyroid is optimized alongside it.
